Family urges Barts Health Trust to address failures after patient's death
The family of a man who died following a series of medical failures in a north-east London hospital have urged the NHS trust to learn more from his case.
Jason Smith, 40, from Wood Green, died from a gastrointestinal haemorrhage at Whipps Cross Hospital.
An inquest found he died of natural causes contributed to by neglect due to a failure by staff to repeat abnormal blood samples and review his condition at the correct times.
Barts Health NHS Trust said it had undertaken a "thorough investigation and actions have been taken to ensure that failings do not occur again".
BBC London's Alice Salfield spoke to Mr Smith's partner Toni Connolly.
