Bristol's major teaching hospital has been told it must improve services in a report by the Care Quality Commission (CQC).
All seven hospitals run by University Hospitals Bristol NHS Trust were visited by inspectors in September.
The report, external found patients were waiting too long for appointments, and staffing levels in operating theatres fell below national guidelines.
Safety and leadership of some services were also said to be needing attention.
The trust says it is already making improvements and some outstanding work was also identified in the report, with children's care and maternity unit singled out for praise.
