  17. 'Missed opportunities' contributed to death, says inquestpublished at 16:21 BST 6 October 2017

An inquest has found that "opportunities were missed" to care for a psychiatric patient who died within hours of being discharged from Weston's Accident and Emergency department.

49-year-old Lisa Lovell died of a blood clot in her lungs on the psychiatric ward, hours later, at the same hospital.

A jury today found that she died from natural causes but there was insufficient communication or joined up care for Lisa's needs.

After Ms Lovell was referred to A&E in the early hours, an agency nurse had filled in a medical chart wrongly and that meant doctors didn't realise the patient was not improving, a report from the hospital found.

She was discharged back to the psychiatric unit.

An independent expert told the inquest that she should have been tested for deep vein thrombosis and that if she had been given the drug Heparin as a result she, "might be alive today".

Her death was recorded as due to natural causes, after missed opportunities.
