It has emerged that details of a family involved in a domestic abuse case, including the personal details of children, were sent to the wrong person by Bracknell Forest Council in late 2017.

The recipient, who knew the family, alerted the council to the breach, and claimed social workers "panicked" when told, and said the council asked them not to share the information with anyone else.

According to the recipient, one report from the ambulance service also contained information about a patient suffering from cancer.

Bracknell Forest Council said a full investigation took place at the time and appropriate action was taken.

  4. 'Unacceptable' accountability on Crossrailpublished at 17:38 BST 3 April 2019

There has been an "unacceptable" lack of accountability over the delays to Crossrail, a report by the Commons' Public Accounts Committee has said.

Crossrail, Europe's biggest infrastructure project, had been due to open in December 2018, but will not now open fully until 2020 at the earliest.

Three emergency cash injections have seen the cost of the route rise from £14.8bn to £17.6bn.

The Department for Transport (DfT) said it "absolutely rejects" claims there was insufficient oversight.

When completed, trains will run from Reading and Heathrow in the west through 13 miles of new tunnels to Shenfield and Abbey Wood in the east.

The practice of Royal Navy submarines flying the Jolly Roger is at the centre of a new exhibition in Portsmouth dedicated to the pirate flag.

The National Museum of the Royal Navy will display a selection of the flags including one seized from a pirate ship in the 1790s.

The original skull and crossbones flag was not coloured black and white as commonly thought but was actually blood red to signify that no mercy would be given once the pirates boarded a ship.

The name of the flag Jolly Roger is thought to have come from the French phrase 'joli rouge' meaning 'pretty red'.
