Anthony Grainger, 36, was shot in a car park in Culcheth, Cheshire, in March 2012 in an operation planned by Greater Manchester Police.

Officers believed the father-of-two was in a group planning an armed robbery.

She said she would expect any document used to brief firearms officers to be "bespoke".
